For in-office visits, you may send your forms ahead of time or bring them with you to your first appointment.  If your first visit is via telemedicine, please email or snail mail your forms.  (SMG email addresses are HIPAA compliant, yet we cannot guarantee the security of all email)

New Client Questionnaire:  Please download, "enable editing" (pop up at top)fill in electronically, then print or save.  Bring in person, mail, or scan and email.

48 Hour Cancellation Policy:  Please print and fill in. Bring in person, snail mail, or scan and email.
